Investigation Summary
At approximately 1:30 p.m. on March 21, 2005, Employee #1 was helping remodel a bathroom on the second floor of a single family residential house. He was demolishing the bathroom floor and walls and loading the debris in a 35-gallon plastic trash can. He then wanted to dispose of the debris in a trash bin on the ground. To do this, he would lower the 35-gallon can from a balcony that was about 10 feet high off the ground. It had a wooden floor of 2-by-6 boards, and it was guarded at the end with a 36-inch-high tubular metal railing. He tried to lower the can of debris to the ground by tying a rope to the can and winding the rope around the top railing of the guardrail. As he lifted the can over the railing, the railing gave way and fell to the ground. Employee #1 lost his balance and jumped, landing on his feet on the concrete below. The impact fractured his right ankle. He required surgery and was hospitalized for more than 24 hours.
