Investigation Summary
At 2:45 p.m. on October 17, 2006, Employee #1 was working as a blending machine operator for a manufacturer of clay tiles, in the ribbon press mixing area. He was inspecting an IMAC Model Number P-250 planetary mixer, with Serial Number 001, when he attempted to retrieve some clay from the mixer housing. The index finger on his right hand was amputated by the mixing blade. He was hospitalized.
