Employee's Thumb Is Amputated in Sheet Metal Brake

At 1:00 p.m. on December 16, 2004, Employee #1, with Santa Rosa Stainless Steel Manufacturing, was forming 10-gauge steel tank legs in an Accupress 12-ft press brake (Model 713012). When the steel material being formed bent upwards, his right thumb became pinned between the material and the press brake ram body. He was hospitalized for an amputation.
