At 9:30 a.m. on March 13, 2000, Employee #1 was unloading a fork lift truck from a flat bed trailer when he somehow fell from the trailer to the ground below, a distance of about 4.5 feet, and was injured. Employee #1 was hospitalized for treatment of his fracture.
