Employee injured when crushed by barrel

At 10:30 a.m. on January 11, 1999, Employee #1 was manhandling a 250-pound barrel of chocolate solids off a pallet and into the work area. As he rolled the barrel balanced on its edge, he slipped and fell, and the barrel fell on him, crushing him. Employee #1 was hospitalized for treatment and released.
