Investigation Summary
At about 5:00 p.m. on October 20, 1998, an employee was working from a scissor lift, adjusting and welding parts of an overhead steel frame canopy. Part of the canopy broke loose, fell, and struck the scissor lift, knocking it over. The employee was thrown out of the scissor lift during the fall and he suffered a fractured hip, for which he was hospitalized.
