Investigation Summary

Investigation Nr: 201121027
Event: 09/11/1998
Employee's Finger Amputated in Machine

Employee #1 was working for an employer that uses roping and garland machines to make artificial Christmas trees. Two of the roping machines were used to make pines from paper and wires, and three garland machines were used to make the twigs, branches, and garlands from the roping materials. Employee #1 was operating a roping machine when she had to un-jam the machine by pulling the wires near the cutter blade area. She suspected that the cause of the jam was that the wires were stuck at the spool. While she was pulling the wires, the cutter blades cycled because she failed to turn the machine off. Employee #1 sustained an amputation to her right forefinger and thumb, and was hospitalized.
