Investigation Summary
At approximately 6:30 a.m. on November 10, 1999, an employee was climbing a ladder to the work platform of a metal scaffold. The platform was approximately 12 ft above the ground. The employee either lost his grip, or his balance, and fell backwards off the ladder to the ground below. He was taken to hospital for treatment and released.
