Investigation Summary
Employee #1 was given a job assignment to look for pipe hangers on the roof of the pavilion under construction by his foreman. Employee #1 was not given any training and instructions about the presence of roof openings and the safeguards necessary to do his assignment safely. The foreman knew of the roof openings and assumed the injured employee knew about them too. Employee #1 fell from the roof. Employee #1 was hospitalized.
