Investigation Summary
At approximately 11:35 a.m. on July 1, 1999, an employee was unbolting the connecting plate of a vertical shore, from a supporting beam. The assembly was lying on its side. As the employee removed the lower bolts of the plate, the assembly dropped, crushing his arm. The employee was hospitalized for a fractured arm.
