Investigation Summary
At approximately 4:00 p.m., 0n July 14, 1997, a three-person maintenance crew was tasked to perform routine maintenance of a milling machine during a scheduled plant shutdown. The process required all three crew members to perform specific functions. The maintenance tasks involved raising the feedroll assembly to facilitate the removing of wires that had wrapped around the shaft. The process required one person at the tower controls to operate the mill, a second signaling the tower, and the third placing a metal block under the feedroll for support. At 6:00 p.m., Employee #1 and a second employee began to raise the feedroll, despite the fact that their supervisor and third crew member instructed them not to begin until he arrived. The two employees attempted to place a six by six-in. by 2-ft long metal flange under the feedroll for support. Employee #1 signaled the second employee in the tower to raise the feedroll while attempting to properly seat the metal flange. The metal flange fell on his thumb, causing a fracture. He was hospitalized.
