Employee's fingers amputated in pleating/shearing machine

Employee #1, of Environmental Filter Corporation, Santa Rosa, CA, was operating a pleating/shear machine used to make filter media when the machine jammed. A coworker attempted to clear it by opening the side and pulling material through. Employee #1 was reaching into the shear section to help pull when an electric eye activated the ram. Three fingers on her left hand were amputated to the second knuckle. Lockout/blockout procedures had not been followed; the employer was cited for a serious accident-related violation.
