Employee injured when struck by wrench on chain drive

Employee #1 was standing in a felt conveyor pan, using a rigid 36 in. pipe wrench to release a stuck or bound chain when he was struck in the head by the wrench. He sustained serious head injuries and lapsed into a coma. Employee #1 did not remove the wrench from the drive shaft before releasing the chain.
