Investigation Summary
Employee #1, age 20, was operating a three-wheeled forklift on a hillside with an approximately 15 degree slope. The site was a staging area for splitting firewood. Employee #1 was killed when he was ejected from the forklift and his chest cavity was crushed. Causal factors in this accident were Employee #1 not being properly trained and his operating the forklift on a sloped surface.
