Employee fractures leg in fall from stepladder on scaffold

Employee #1, who worked for a painting subcontractor, was spray painting the underside of a roof overhang at a school remodeling project. He was using a patent-type steel, welded-frame scaffold with cross-bracing and locking, rubber-tired wheels. Employee #1 placed a 6 ft stepladder on the 71 in. high scaffold platform and climbed the ladder to access the work area. The ladder fell to the left, causing Employee #1 to fall onto the concrete. He fractured his left femur and was hospitalized for six days. Causal factors for this accident include using a ladder on top of the scaffold platform and scaffold brakes that were not effectively locked.
