Employee injured in fall when extension ladder slips

Employee #1 was standing on an aluminum extension ladder, stapling a moisture barrier to the exterior frame of a building at a construction site. He leaned too far to one side and the ladder slipped. Employee #1 fell approximately 12 ft. and sustained soft tissue damage to his lower back and shoulder.
