Employee's toes injured when run over by gondola

Employee #1 was picking grape leaves from a moving gondola loaded with grapes that was coupled to the back of a narrow gauge agricultural crawler tractor. Her foot was run over, amputating one of her toes and lacerating another. There were no witnesses and the cause of the accident was not clear.
