Employee injured in fall from moving joists

At approximately 1:15 p.m. on July 27, 1999, Employee #1, a journeyman carpenter, was installing joists. He was wearing a safety harness and a safety line had been installed, but he was not hooked onto it. Employee #1 stepped on diaphragm plywood edge blocking and went through the third-floor joists on a 20 1/2 in. wide bay, falling approximately 14 ft 6 in. He sustained fractures to his left leg and wrist, and underwent surgery at San Francisco General Hospital. The employer should implement and maintain a written Injury and Illness Prevention Program (IIPP) that addresses the hazards associated with materials handling and work surface layout and condition. The employer in this situation conducted intermittent meetings, but a comprehensive safety training program did not exist. The employer was cited for a violation of T8CCR 3203(a)(7)(b), the employer shall provide training and instruction to all new employees and to employees given new job assignments for which training has not previously been received. It was also recommended that the employer implement a safe materials handling process and associated training programs.
