Employee injured after falling from wall

At approximately 8:50 a.m. on December 29, 1998, Employee #1 was working with two coworkers on the roof of a building located in San Francisco, California. Employee #1 and two coworkers were removing a formwork from a 60.75-in.-high wall, which had just been poured with cement. When one of the coworkers asked Employee #1 to help him remove a form that was stuck, Employee #1 got up on top of the wall to pry the form from the top. At the time of the accident, Employee #1 was sitting on a concrete wall and prying the wood away from the wall. Suddenly, the form broke loose and Employee #1 fell backwards off the wall, hitting the roof and landing on the ground. Employee #1 sustained cuts and lacerations to the head. He was taken to San Francisco General Hospital for treatment.
