Investigation Summary
At 12:45 a.m. on Augsut 7, 2017, an employee was using a standing reach truck to move pallets onto the floor for overnight stocking. The model industrial truck was a Yale Lift Truck Model #NR040DBNS24TE091. While turning down an aisle, the lift truck slid towards the corner of the racks placed along the wall. The employee had his foot crushed between the rack and the lift truck.
