Investigation Summary
Employee #1 was a wrapping foreman for Interstate Brands Corporation and supervised coworkers on a production line of eight machines. At approximately 1:30 a.m. on April 10, 1998, she was installing a new roll of wrapping paper on the Dough Boy #6 baked goods wrapping machine. To feed the paper, she reached into a hood-type guard with her left hand, amputating the end tips of her index, middle, and ring fingers. The cause of the accident was Employee #1 attempting to load the new roll of paper with the machine running.
