Investigation Summary
At approximately 9:27 a.m. on September 27, 1997, Employee #1 and two coworkers were installing roof decking when he noticed that an area of the decking had not been tacked. Employee #1 was walling in the area of loose decking when he fell approximately 10 ft to the grade. He sustained injuries that required hospitalization. At approximately 10:10 a.m. on September 23, 1997, Employee #2 was offloading I-beams from a sling hooked to a crane when the crane operator told him that he was going to lift the load. Employee #1 removed his hands from the sling, then reached in again with his left hand. The shifting I-beams amputated the tip of his little finger.
