Investigation Summary
At approximately 12:14 p.m. on October 19, 2009, an employee was working on a bathroom roof, placing a bucket to capture rain water leaking from above, large warehouse-type building with roof height of about 20-ft. He fell through the roof, drywall with 24-in. O.C. beams, onto a toilet, sustaining contusions to his head and back. The employee was hospitalized and treated for concussion.
