Investigation Summary

Event: 08/11/2009
Employee's Finger Is Amputated by Press

At approximately 6:45 a.m. on August 11, 2009, a supervisor at a Santa Rosa, CA tile manufacturing company, was demonstrating to a repair technician an air purge malfunction on a clay cutting machine cavity press ("Cookie Cutter"). The supervisor placed his hand into the stamping cavity to show the lack of pressurized air, when the press came down and cut his hand. The supervisor's right index finger was amputated to the first joint, below the knuckle. He was able to withdraw his right hand after he pressed a reversing control button with his left hand. An air ambulance transported the supervisor to Healdsburg Hospital, where he was treated for the amputation and released within three hours.
