Investigation Summary

At approximately 9:00 a.m. on October 14, 2010, Employee #1 was reporting to work at the winery, carrying his lunchbox in one hand and his work boots in the other. He was on the second or third step of the six step flight when his foot slipped backward, causing him to stumble forward and strike his knee on the next step. He was driven to a nearby clinic and later admitted to St. Helena Hospital for surgery on his fractured left femur.
