Employee's Foot Is Injured When Run over by Forklift

At approximately 6:06 p.m. on September 8, 2008, Employee #1 was operating a RAYMOND Model DSS 350TT forklift. He was training to drive the forklift around an obstacle course of traffic cones in an open asphalt-paved loading area. He was driving the unit backwards with his right foot on the dead man pedal when he lost his balance and staggered backwards. His left foot stepped on the ground and was run over by the rear wheels of the vehicle. He sustained severe degloving of the right foot and was hospitalized.
