Investigation Summary
At approximately 9:45 a.m. on December 17, 2010, Employee #1 was repairing a semi-automatic Corking Machine, GAI, Model Number 405. Employee #1 should have shut off power to the corking making machine before beginning work. After installing a new limiting switch, Employee #1 manually adjusted the timing by rotating a motor-driven belt and pulley with his left hand. The motor activated, and the left middle finger of Employee #1 was caught between the belt and pulley. Employee #1 was hospitalized. The injury resulted in an amputation at the first knuckle.
