Employee killed when struck by falling bail from ladle crane

When Employee #1 finished pouring molten metal from the ladle crane, he lowered the ladle and unhooked it from the bail. He was removing the handle that releases the metal when the bail fell, striking Employee #1 on the head. He was killed. No employee was responsible for making sure the bail was secure after it was unhooked from the crane.
