Investigation Summary
At approximately 7: 30 a.m. on or about August 22, 1997, Employee #1 was operating L & J partial revolution press #112, using pullbacks to protect his hands when in the dies and a single-guarded foot pedal to trip the press. He was reaching into the dies after the stroke to remove the part when the press cycled, crushing his right hand and amputating his index finger. Evidently, the two-hand pullbacks were not adjusted properly. At approximately 4:30 a.m. on or about September 10, 1997, Employee #2 was finishing a setup on L & J partial revolution press #131. He was using a single-guarded foot pedal to trip the press while feeding rolled steel into the progressive die, having bypassed the interlocked barrier guard. He was reaching into the die to adjust the steel strip when the press cycled or jogged. His left hand was pinched in the dies and all four fingers had to be amputated. Two-hand controls were not being used or properly aligned to prevent Employees #1 and #2 from having their hands in the point of operation during the adjustment stroke.
