Event: 11/19/1998
Employee's finger injured in power press

At approximately 1:00 p.m. on or about November 19, 1998, Employee #1, a set-up person, completed a repair and adjustment on the left pull-back assembly of a 56 ton Johnson full-revolution mechanical power press, and he began operating the machine. Sometime between 1:00 p.m. and 2:00 p.m., part of the pull-back assembly separated, and he repaired it. Around 2:00 p.m., Employee #1 inserted a part in lower die tool with his right hand, hit the foot pedal and began to remove the part with his left hand as the upper die tool moved down to stroke the part. The part bent against the upper die, catching and injuring Employee #1's left middle finger between the part and the die. He was transported by ambulance to Deaconess Hospital, where he was treated and released later that day. Employee #1 was wearing pull-backs on both hands at the time of the accident.
