Investigation Summary
At 12:30 a.m. on July 19, 2017, an employee using a forklift that was a dock plate that was between the dock and railcar to unload freight. The employee was exiting the railcar in the forklift and the dock plate collapsed and the forklift fell into the gap between the railcar and the dock. The employee was pinned between the dock plate and the forklift. The employee was transported by ambulance to St. Mary's Hospital in Long Beach, CA, where he was treated for a broken leg.
