Investigation Summary
At approximately 8:55 p.m. on August 1, 2017, an employee was operating an ergonomic J Fold machine in order to lift the patient gowns closer to his hands. The employee thought the motor was not working properly to move the roller in back. The employee went to the back of the machine, after jamming the toggle switch into the on position, to try to release the roller. The employee got caught in the roller while attempting to release a stuck roller. The employee suffered several broken bones in his left arm for which he was hospitalized.
