Employee killed when pinned under skid steer loader

Employee #1 was operating a Case uniloader. He had a bucket load of dirt and had dumped part of it off the edge of the driveway. With the bucket elevated, he was driving down the driveway to dump the rest of the dirt in another pile when he ran over a small rock and lost control. The bucket dipped downward, throwing him off the loader. The machine ran over him and came to rest with the bucket on the ground and the rear wheels high in the air. Employee #1's back was pinned against the pavement in the area between the loader's front wheels. He was killed. The vehicle's seat belt was missing its buckle.
