Investigation Summary
At approximately 2:00 p.m. on April 21, 1997, Employee #1 was helping a coworker who was adjusting the front elevator door mechanism on top of the elevator car. Employee #1 was crouched over the door operator, on top of the car on the southeast side, with his back to the elevator shaft's south wall. The elevator car unexpectedly moved up, causing Employee #1 to lose his balance and fall backward into the 13 1/2 inch wide open space between the car and the shaft wall. The car continued moving up from the basement to the first floor, where it stopped. Employee #1, having been dragged upward by the car, was caught about 4 ft below the car top within the open space between the car and the shaft wall. The emergency rescue team arrived about 13 to 14 minutes later, and removed Employee #1. He suffered multiple internal injuries and died on April 25, 1997. No fall protection had been provided, and lockout/tagout procedures had not been followed.
