Investigation Summary
At 10:30 a.m. on July 28, 2017, an employee was attempting to clear a clog on the hot fill machine. He disconnected hoses to see if the lines were clogged. He looked into an actuator where the hose had been connected and thought he saw something inside that may have been causing the clog. He stuck his finger into the actuator housing to try and clear the clog. He had not shut the machine down prior to sticking his finger in the actuator housing. The actuator cycled while the employee's finger was in the housing. The tip of the left index finger was amputated. The employee was not hospitalized.
