Investigation Nr: 170386197
Event: 11/08/1996
Employee killed when struck by falling aerial lift mast

Employee #1 and a coworker were preparing to move a Sky King Aero 60 aerial lift through a doorway. The lift would not roll through the doorway with its mast fully lowered. The coworker, who had no understanding or knowledge of the tilt mechanism, disengaged the spring cylinders that supported the lift when it was tilted and then tried to tilt the lift. The mast of the lift fell to the floor, striking Employee #1 on the head. He was killed.
