Investigation Summary
At approximately 9:25 a.m. on August 5, 2017, an employee was operating an Underhung Swing Cut-Off Saw (Whirlwind, Model-212, Serial Number. 671098ZL) to cut 8 feet by 1.5 inch by 0.5 inch particle board into different sizes. As he was cutting the particle boards, he tripped over a mat on the floor and accidentally depressed the foot pedal. The employees left index and middle finger struck the blade causing amputation of his index finger and laceration on his middle finger. The employee was taken to the Corona Regional Medical Center by ambulance where he was treated and released after about 6 hours.
