Employee killed in fall through skylight

At approximately 10:30 a.m. on July 14, 1999, Employee #1, of Abatemaster Environmental, Inc., was at a height of approximately 16 ft on the built-up roof of an abandoned industrial site slated for demolition. He was part of a 12-man crew removing the approximately 60 ft by 130 ft rubberized flat roof surface, which contained asbestos. Employee #1 and a coworker had just started a regular morning break and were walking together to get water and a drink at a spot near a 42 in. square domed skylight in the middle of the roof. As the coworker was opening a package of drink cups, Employee #1, who was behind him, flopped back onto the approximately one-eighth in. thick opaque acrylic or plexiglass-type dome, as if to rest. He fell 16 ft through the skylight onto the concrete floor. Employee #1 was transported to the hospital, where he died at 12:23 p.m. on July 15, 1999.
