Investigation Summary
Employees #1 through #5 were involved in the replacement of a defective water can below a furnace containing molten glass. Employees #1 and #2 were tapping the water can immediately below the furnace floor and Employee #5 was observing them. Employees #3 and #4 were manning fire hoses. After the molten glass had partly drained from the furnace, molten glass erupted from beneath Employees #3 and #4. Employees #1 and #2 were hospitalized for 3rd-degree burns to their heads, necks, faces, arms, backs, and shoulders. Employees #3, #4, and #5 suffered first- and second-degree burns to their hands, arms, and faces.
