Employee dies after fall from unguarded mezzanine

On September 14, 1994, Employee #1 was found unconscious on a concrete floor by other employees. He had apparently fallen approximately 13 feet from a mezzanine. Employee #1 was hospitalized immediately after the accident and remained in the hospital until his death on October 14, 1994. No autopsy was performed. The cause of death was listed as right temporal skull fracture with hemorrhage due to a traumatic fall from a height, with other conditions of respiratory and renal failure. No one saw the accident, but investigation showed that the fall occurred because Employee #1 was required to lean out in order to place clean clothes in sequence for conveying to delivery trucks. No guardrail was present.
