Investigation Summary
At approximately 7:00 p.m. on November 7, 1994, Employee #1 was having a conversation with a forklift driver while the forklift was moving. Employee #1 was walking alongside the lift truck on its right side, between the truck and a stack of wood panel stock. The driver made a left turn and the rear end swung and crushed Employee #1 between the forklift and the stack of wood. He was taken to N.C. Baptist Hospital in Winston-Salem and died there at approximately 2:00 a.m., on November 8, 1994.
