Investigation Summary
Two employees were replacing the roof on a furniture company warehouse. The roof was 6.7 meters above the ground. The employees gained access to the roof by means of an 8.3-meter aluminum ladder. The ladder was leaning against the southwest side of the building, about 1.8 meters from a 13-kilovolt, three-phase overhead power line that was 6.9 meters above the ground. The two employees stood the ladder in an upright position to lower it to the ground. The base of the ladder slipped in the wet grass, and the top of the ladder contacted the power line. Both employees sustained electric shocks and burns to their hands and feet and suffered cardiac arrest. One of them was revived by emergency medical services. He was hospitalized for his injuries. The other employee was electrocuted.
