Employee killed in excavation cave-in

At approximately 2:30 on November 29, 1995, Employee #1 was in an 18 ft deep excavation fitting together sections of pipe for a toe drain around a reservoir. The east wall of the excavation caved in, trapping or covering Employee #1 and three of his coworkers with soil. The coworkers were able to dig themselves and each other out and suffered no appreciable injuries. Employee #1 could not be rescued in time and died of the injuries he sustained during the cave-in. The sheer wall sloughed off due to inadequate sloping, wet conditions, and type C soil.
