At 9:35 a.m. on July 21, 2017, an employee was greasing some fittings in HVAC ma chine #37 unit. The employee was existing the unit and slipped, causing his han d to reach out to catch his fall. The employee's hand landed on a belt and was pulled into a pulley. The employee amputated the tip of his left pinky finger.
