Investigation Summary
At 12:20 a.m. on July 28, 2017, Employee #1, a supervisor, was using a stand-up forklift, Number 34, Yale Model 0S030BEN24TE119 and Serial Number D826N03471G, to pick up a boxed door for transport at aisle 63 of the warehouse. The boxed door was not secured on the forklift prior to moving. Employee #1 drove the stand-up forklift into the upright of the storage rack, which caused the unsecured door on the pallet to strike his lower leg as he stood on the operators platform. The employee suffered a fractured lower leg (ankle area) and was transported to a local hospital, where he was hospitalized for two days.
