Investigation Summary
At 4:00 p.m. on July 24, 2017, an employee was performing powered industrial equipment training for pallet jacks. The employee leaded to the right while returning the electric pallet jack to the trainer and stepped from the jack while it was still moving. The employee released the dead man switch and pinned her left foot under the lift. The employee suffered several broken bones in her foot and received a laceration. The employee was hospitalized for one day.
