Investigation Summary
At approximately 12:45 p.m. on July 13, 1995, Employee #1 was part of a five-man crew cleaning and patching a section of pavement on I-75 and Clark Street. Orange barrels were installed and a traffic lane was closed. Employee #1 was spraying emulsion on the patched areas when he was struck by a dual wheel tire with part of an axle attached to it. He died at Detroit Receiving Hospital. No citations were issued.
