Investigation Summary
At 2:45 p.m. on July 17, 2017, an employee was installing insulation into the attic. The employee was standing on a 2 by 4 board that was not secured properly and fell approximately 9 feet when the board loosened. The employee landed on his feet and collapsed to the ground right away. The employee suffered multiple injuries to include several ruptured disks in the mid to lower back, a broken spinal cord, tendon tears in the right arm and hand, and sustained lacerations to the left hand. The employee was hospitalized.
