Employee dies in fall from 7th floor of power plant

At 4:55 p.m. on March 23, 1995, Employee #1, of RMF Inc., and another boilermaker were assigned to repair boiler tube membranes on the 7th floor of the River Rouge Power Plant. The coworker entered the boiler and Employee #1 remained outside. When the coworker attempted to start welding, he found that his welder would not function and sent Employee #1 to trace the welding lead and find the problem. Afterward, the coworker started working. Soon thereafter, Employee #1 was seen hitting the 4th floor. It appears he climbed onto the top of a gangbox, then onto the permanent guardrail, and fell while moving the welding lead which was shorting out. He died. Citations were issued for violations of R408.40114(1), R408.40631(1), R408.40743(4), R408.40747(3), R408.40744(7), and R408.22311(1).
