Investigation Summary
On May 24, 2017, an employee was installing an additional emergency stop button on the stacker machine and accidentally dropped an electrical box inside the machine. The employee attempted to reach into the machine to retrieve the box and his arm was caught by the flipping bucket against the machine's frame. The employee's arm skin was stripped off when it was caught by the flipping bucket. The employee was hospitalized.
